How well any ad campaign performs depends on things no recipe can control: your budget, how crowded and expensive your industry's auctions are, the products you sell and what you charge for them, how well your website actually converts visitors, the quality of your customer support, your reviews and reputation, your creative, and plain timing. A perfect recipe on top of a weak offer or a broken website will still lose money — ads amplify what's already there, good or bad.
The budgets shown are suggestions only — yours could and should vary with your available funds and your goals. Follow this recipe with caution: make sure every step lines up with your budget and your business, and edit where necessary. We explain why each step exists, but nothing here is one-size-fits-all. Before you spend a dollar, be blunt with yourself about the list above — get your house in order, then use the recipe to learn and test. That's what it's for.
Check the rules that govern your industry before you run anything. Plenty of businesses carry advertising obligations that have nothing to do with the ad platforms: licensing boards, professional advertising standards, rules about what you may claim and how you must substantiate it, age restrictions, and consent laws covering email, texting, and calling. Those rules are yours to know, they vary by state and country, and they override any step in this recipe.
If you work in a licensed or regulated field — health and medical, legal, financial and lending, insurance, real estate, alcohol, firearms, cannabis, childcare, education, anything with a board or a license behind it — confirm what applies to you before you spend a dollar, and ask your attorney or your licensing body if you are not certain. This recipe uses Google Search as its single platform, starting with conversion tracking and a tightly scoped local Search campaign before anything else. Search is the right call for medical clinics because patients already know they need care — they’re typing it into Google right now — so you meet demand rather than manufacture it. The sequence is: nail tracking first, run a lean exact/phrase-match Search campaign to capture near-ready patients, then graduate to Performance Max once you have 30+ conversions to guide it.
This recipe is for independent urgent care centers, primary care practices, specialty clinics, and similar outpatient facilities with one or a handful of locations. The core problem is that most clinics either skip tracking entirely (so they never know what’s working) or blow budget on broad keywords that attract job-seekers and medical students instead of patients. Both mistakes are fixable with discipline from day one.

Tracking Foundation & Healthcare Policy Compliance
Google Ads conversion tracking (not GA4 imported goals): Install the Google Ads tag via Google Tag Manager. Create two conversion actions in Google Ads: (1) Phone call from ad — use the Google forwarding number, count every call, 60-second minimum duration; (2) Appointment form submission — tag fires on the thank-you page URL. Set both as Primary conversions. Do NOT use GA4 imported conversions as your sole source — they lag and misattribute.
Healthcare ad policy check (required before launch): Google restricts personalized advertising for healthcare and medical content. You must: (a) confirm your ads do not use remarketing audiences built on health conditions or sensitive medical interests; (b) review Google's Healthcare and Medicines policy — certain prescription drug terms and some treatment keywords require certification or are prohibited; (c) if you plan to advertise specific prescription services, apply for Google's healthcare certification. For a general clinic running appointment-focused ads, standard Search campaigns are permitted, but you cannot layer audience targeting based on inferred health conditions.
Call tracking: If you use a third-party call tracker (CallRail, etc.), map it to the Google Ads phone conversion so the data flows back into the platform.
Before spending a dollar on ads, make sure every phone call and form submission gets recorded in Google Ads itself — not just in Analytics. This phase also forces you to read Google's healthcare ad rules so you don't get your account suspended on day one. It's unglamorous but skipping it means you'll never know which keywords are actually booking patients.
New to "funnel" talk? What top, middle, and bottom mean
Think of every person who could ever buy from you as water moving through a funnel. Top of funnel is strangers — people who've never heard of the business. Campaigns here buy attention and first visits, not sales, so expect cheap clicks and few purchases. Middle of funnel is people who've shown interest — they visited, engaged, or are comparing options. Campaigns here re-engage and build the case. Bottom of funnel is people ready to buy right now — searching for exactly what you sell or abandoning a cart. Campaigns here convert; the clicks cost the most because the intent is worth the most. Full funnel means automated campaigns that work every stage at once. One thing that surprises people: recipes usually start at the bottom — capture the demand that already exists before spending money creating new demand at the top.
Local Intent Search Campaign — Appointment & Urgent Care Keywords
Launch a single Search campaign targeting a tight geographic radius (5–15 miles around the clinic, or by city/zip). Use Manual CPC to start — set bids at $4–$8 and raise only on keywords that generate calls or form fills. Switch to Maximize Conversions only after you have 15+ conversions recorded.
Ad policy note: Do not make claims you cannot substantiate ("best doctor in [city]", "guaranteed results"). Avoid before/after imagery. If advertising any controlled substance treatment, additional certification is required. Keep ad copy factual: services offered, hours, insurance accepted, same-day availability.
Structure: one campaign, 2–3 ad groups split by intent cluster (e.g., Urgent Care, Primary Care, Specialty if applicable). Each ad group gets 2 Responsive Search Ads with 10–12 headlines and 4 descriptions pinning the clinic name and location to positions 1–2. Enable call assets and location assets on every ad group.
Targets people actively searching for a clinic or appointment in your area — the closest thing to a patient already walking to your door.
- 1. In Google Ads, click + New Campaign → goal: Leads → campaign type: Search.
- 2. Name it 'Search — Local Clinic [City]'. Uncheck Search Network partners and Display Network.
- 3. Bidding: choose Manual CPC, enable Enhanced CPC OFF (keep it manual). Set a $30–$50/day budget.
- 4. Locations: enter your city or set a radius of 5–15 miles around the clinic address. Targeting: People in or regularly in this location. Exclude: People searching about this location (to avoid out-of-area researchers).
- 5. Languages: English (add Spanish if your clinic serves Spanish-speaking patients).
- 6. Create Ad Group 1: 'Urgent Care [City]' — paste in the urgent care keyword list (phrase and exact match only, no bare broad match).
- 7. Create Ad Group 2: 'Primary Care / Family Doctor [City]' — paste in the primary care keyword list.
- 8. In each ad group, create 2 Responsive Search Ads. Pin clinic name or city to Headline position 1. Include call-to-action in at least 3 headlines (Book Appointment, Same-Day Visits, Call Now).
- 9. Add assets: Call asset (use Google forwarding number), Location asset (link Google Business Profile), Sitelink assets (Services, Hours, Insurance Accepted, Book Online).
- 10. Add all negative keywords from the starter list at the campaign level before enabling the campaign.

This is your main patient-acquisition campaign. It only shows ads to people within driving distance who are actively searching for a clinic or appointment — the highest-intent audience you can reach. You're bidding manually at first so you don't overpay before you know which keywords convert. The ad groups keep urgent care searches separate from primary care searches so you can see what's working and adjust bids accordingly.
Search Term Audit & Bid Refinement
At the end of weeks 2 and 4, pull the Search Terms report (Keywords → Search Terms). For every term that spent more than $15 with zero conversions: add as negative or tighten to exact match. For terms converting at an acceptable cost-per-lead: note the exact phrase and consider adding it as an exact-match keyword with a higher bid. Adjust ad group bids based on which cluster (urgent care vs. primary care) is generating calls.
Check impression share (lost to budget vs. lost to rank) in the Auction Insights and Campaigns columns. If lost to budget is above 20%, increase daily budget before raising bids. If lost to rank is high, raise bids on your best-converting keywords first.
This is also when you QA conversion tracking: spot-check that test form submissions and calls are appearing in the Conversions column within 24 hours.

Two weeks in, you dig into the actual search terms that triggered your ads — not the keywords you bid on, but what people literally typed. You'll almost always find wasted spend on irrelevant terms (job seekers, students, people in other cities). Cutting those and doubling down on what's converting is where most of the early efficiency gains come from. This isn't a one-time task; do it every two weeks for the first three months.
Performance Max Campaign — Unlock After 30 Conversions
Once your Search campaign has logged 30+ conversions (calls + form fills combined) over any rolling 30-day window, layer in a Performance Max campaign. PMax will extend reach to Google Maps, Display, YouTube, and Gmail — useful for local awareness — but it needs real conversion data to optimize; launching it cold wastes budget.
Healthcare policy note: PMax asset groups must comply with the same healthcare content restrictions as Search. Do not include testimonials that make specific outcome claims. Avoid imagery of medical procedures that could be considered graphic. Google may limit certain audience signals for health-related advertisers.
Set PMax budget at $15–$20/day (separate from your Search campaign budget). Provide one asset group per service line. Upload audience signals: your existing patient list (hashed emails, if HIPAA-compliant data handling is confirmed with your legal team), plus in-market audiences for 'Health & Fitness → Health Conditions' and 'Local Services.' Set a Target CPA equal to your acceptable cost-per-new-patient-lead from the Search campaign data.

PMax spans the full funnel — it re-engages past site visitors, reaches in-market audiences, and surfaces the clinic on Maps and YouTube — but only works efficiently once conversion data guides the algorithm.
- 1. In Google Ads, click + New Campaign → goal: Leads → campaign type: Performance Max.
- 2. Name it 'PMax — Local Clinic [City]'. Set budget: $15–$20/day.
- 3. Bidding: Target CPA. Enter the CPA figure from your Search campaign data (total spend ÷ total conversions). If CPA data is thin, start with Maximize Conversions and add a Target CPA after 2 more weeks.
- 4. Locations: same radius as your Search campaign. Languages: match Search campaign.
- 5. Asset Group 1: name it after your primary service (e.g., 'Urgent Care'). Upload: 3–5 images (clinic exterior, staff, waiting area — no graphic medical imagery), 1–2 logos, 1 landscape video if available (15–30 sec), 5 headlines, 5 long headlines, 5 descriptions. All copy must be factual and policy-compliant.
- 6. Audience Signals: add your patient email list (confirm HIPAA-compliant handling before uploading), plus Google's in-market segment 'Health & Fitness' and a custom intent audience built from your top-converting Search keywords.
- 7. Final URL: send to your appointment booking page or a dedicated landing page — not the homepage.
- 8. Enable ad extensions: call, location, sitelinks (same as Search campaign).
- 9. Set campaign start date. Do NOT pause your Search campaign — let both run simultaneously; Search captures high-intent queries with more control, PMax handles broader reach.
- 10. Review PMax asset performance labels (Best, Good, Low) after 3 weeks and replace 'Low' assets.
Performance Max is Google's automated campaign type that runs across all Google surfaces at once. It's powerful but needs data to work — if you launch it with no conversion history, Google's algorithm guesses randomly and burns your budget. Wait until you have 30 real conversions, then turn it on with a modest separate budget. Think of it as an amplifier for what's already working, not a replacement for your Search campaign.
Measurement & Ongoing Discipline
Weekly checks (15 min): Conversions column — are calls and form fills recording? Cost-per-conversion trend — flat or improving? Search Terms report — new negatives needed?
Monthly checks: Compare cost-per-lead by campaign (Search vs. PMax). If PMax CPA is more than 2× Search CPA after 60 days, reduce PMax budget and investigate asset quality. Check Google Business Profile call volume and direction requests as a proxy for offline impact.
What this recipe cannot control: Your cost-per-lead is heavily influenced by (a) how competitive your local market is — a clinic in a dense metro with 10 competitors will pay 3–5× more per click than a rural clinic; (b) whether your landing page or booking flow converts — a page with no online booking, slow load times, or no trust signals (reviews, credentials, hours) will waste every click regardless of ad quality; (c) your offer — same-day availability and broad insurance acceptance are the two factors that most reliably improve conversion rates for clinics. Fix those before blaming the ads.
Retargeting note: At $30–$60/day total, your site traffic will be too thin to run a meaningful retargeting campaign. Once your Search + PMax campaigns are driving 500+ monthly site visitors, you can layer a remarketing audience into PMax audience signals or launch a small Display retargeting campaign as a separate unlock.

Ads are not a set-and-forget system. The weekly 15-minute check keeps you from bleeding money on bad search terms or broken conversion tracking. The monthly review tells you whether PMax is pulling its weight or just spending. And the honest note at the bottom is worth reading twice: if your website is slow, your booking form is buried, or you're not accepting new patients, no ad campaign fixes that — the recipe only works if the clinic itself is ready to receive patients.
